Maintenance Planning and Graduation
Congratulations on completing your treatment in Collaborative Care! Reducing behavioral health symptoms can be challenging, but your hard work and dedication has paid off. This phase of treatment is called Maintenance because the goal is for you to maintain the progress you have made by continuing to work on the skills you have learned. It is a huge accomplishment to have made it this far and this journey provides support and education to guide you as you move forward with your life.
Congratulations on making it to the Maintenance phase of your treatment. This means that through hard work and dedication, you have reached the goal of reducing your behavioral health symptoms.

How Do You Feel?
Taking steps toward discharge from active treatment can bring on a range of emotions. Getting these things out of your head and written down can help you process and plan. Take a minute now to make a journal entry discussing your thoughts and feelings about completing treatment.
When to Return to Care
As with all chronic conditions, mental health symptoms may return. Being aware of small changes in your mood, anxiety, stress-level, and sleep can help you identify symptoms so you can take action to get yourself back on track, whether that means increasing your self-care practice or returning to treatment.

Goal Setting
During your time in Collaborative Care you and your Care Manager set goals and worked toward achieving them. Take a few minutes to complete this SMART Goal Setting Worksheet, so that you can keep making progress toward things that are important to you.
Practice Mindfulness
Staying in the habit of practicing self-care and maintenance activities can be challenging at first, especially when you no longer have a Care Manager to keep you on track. The more you practice these positive coping skills, the more likely they are to become a normal part of your routine.

One Positive Thing You Have Noticed Since Completing Active Treatment
Take a moment to reflect on the progress you made in Collaborative Care. Write a short journal entry about one positive thing you have noticed in your life since completing active treatment.
